Grade Level Rules

Team Rules

1. Come to class prepared

2. Follow all directions first time they are given

3. Respect yourself, other students, faculty, and equipment of DHMS

4. Stay in your seat and talk only with the permission of the teacher

5. No eating, drinking, or gum chewing allowed in class

Team Discipline

If a student chooses to break any of the rules set forth by the 5^th grade team teachers, the following are the consequences:

1. Warning
2. Conference
3. Lunch Detention
4. Phone Call Home
5. Team Conference
6. Referral to the office

The teacher has the right to by-pass any consequences based on the severity of the situation.

1. Positive note to student
2. Positive note/phone call to parent
3. Reward certificate/Incentives

Tardy & I.D. Policy

1^st offense-Warning given by teachers

2^nd offense-Warning given by teachers

3^rd offense-Parent contacted

4^th offense-Parent contacted/teacher consequence

5th offense- Referral

6th offense- Referral/ 1 day ISS

7th offense- Referral/ 2 days ISS

8th offense- Referral/ 3 days ISS

9th offense- Referral/ 4 days ISS

10th offense- Referral/ OSS
